This form is for the HEAD/s OF HOUSEHOLD, typically a single adult or a married adult and spouse. When you submit this form you will be redirected to the NEW PARISHIONER REGISTRATION page where you can click on the OTHER HOUSEHOLD MEMBERS form and the CHRISTIAN FORMATION REGISTRATION form accordingly.

A member of our Welcoming Committee will contact you to review your completed form and answer any questions you might have. If there is more information that you could/did not provide at the time, we will inquire about it then.
